    Hi all,

    I'm new to JotForm, coming from Google Forms, and I'm considering using this for a Rabies Clinic we're holding. I need to gather Owner information, pet information, and schedule appointments. I think I'm doing fine with the former and latter, but I'm having a little trouble with gathering pet information, which I was hoping you'd be able to help with. I have clients who will come with multiple pets, and so I'd like to collect info for all of them, but the form should depend on how many pets they have, so I was wondering if there was an elegant way to do that within JotForm.

    The other question I had (which might be significantly harder), is whether there was an easy way to take this info collected in the forms, and export it out to a fillable boxes in PDF file, ideally in a bulk fashion. I have a significant amount of technical experience, so I'm not afraid to get my hands dirty if necessary. :)

    Please try our infinite list or configurable list widget? You can add dynamic answers based on the number of pets they have.
