What happen to my forms? My forms are saying not found

Your form is disabled because your account is suspended. The account is suspended because your Chitex Dispatch Carrier Profile form is used to collect credit card info with plain text fields, which is against our Terms of Use.

I deleted the quilty fields and activated your account.

If you want to process payments, you need to use one of our payment processors. And if you want to charge your users at a later date, you can enable payment authorization.
