Hello,
Our organization, thedacare.org uses jotform for one of our online forms. We are in the middle of launching a website redesign, and need to make changes to the jotform, but no one within the organization knows what the login credentials are for our account. The url provided below is of our new website, the form is located at the bottom of the page, or by clicking the button in the blue header to jump down to it.
Is there any way you can provide the email address that is on file with our account - I'm not even sure how to identify our account, other than the fact that the email address should have "@thedacare.org" in it. Or if it's possible to send a password reset to both the email account on file and my email, so I can contact the person within our organization (if they're even still employed here)
I greatly appreciate your help.

The form that you provided is under the account "EruptrJotForm" and the email is "t**f.**b@gmail.com"
