Good morning, I have been using Jott form and I really like it. I was wondering if there is a way where we can have a multiple-choice of forms to display at the beginning. For example I have multiple people that need to fill out different type of information but I want to use one new one device to collect that data. I run a restaurant and each part of restaurant needs to enter different data assigned to different user I want to make it as seamless as possible to collect the checklist. For example at the end of the day I need each staff to complete a checklist. I know I am using the trial version. Maybe it might be available on the upgraded version thank you and please let me know what do you think I am really interested in upgrading the service

    Hello Lhakpa,

Thank you for contacting JotForm Support.

If I am understanding your objective correctly, your goal is to have different staff members complete different parts of the same form, correct?

If so, then this can actually be done by including multiple sections on a form and then using the form conditions to hide or show certain sections of the form based on who is filling in the information.

For example, you can have a form that has a section dining room staff and another for the kitchen staff. Then, you can add a question that asks who is filling the form "Kitchen or Dining" and based on the answer, the form will display the corresponding section.

Here is a sample form I created to replicate this functionality - https://form.jotform.com/202956952587067
