User friendly seach form

Using Jotform,

There the a user friend form you can create to return a result. After the database has thousands of records.

The database has the fields ID, Name, Address, DOB

Using a user friendly front end type in a name and it returns the results of all record matching that name.

Unfortunately it is not possible to create a form to search database based on any text. I would also suggest you to please take a look at our "Spreadsheet to form" widget and see if that fits into your requirement.

You can upload an Excel file with all the data and a unique column data that could be the ID field. When the user input their ID in widget on form, it will fetch the existing data from the uploaded Excel file and display in form fields.
