Billing/Insurance form is not loading on website

    Hello,

    I created links to forms on my client's website and the Insurance Billing form does not appear to be loading for her properly when she goes to test it. It either goes to a log-in page or doesn't load at all. It does appear to work for me. I had a friend try it out as well and they said it wasn't loading for them. What could be causing this issue? I've included the url for my client's website. The form having issues is 1.BILLIING/INSURANCE FORM.

    I used the link provided in the PUBLISH section of the EDIT FORM.

    I tested the form and it is loading just fine on my end. I also see the login/signup page which is normal for a private form.

    Just FYI, the reason why she and all your users like on my test would get the login/signup page is that your form is set private. This means that only invited users or logged-in users can view your form. You have to set it back to Public so all your users would be able to access the form directly without logging in or creating a JotForm account.

    I also cleared your form cache, could you please ask your client to try it again? If the problem persists, let us know.
