Survey form for mobile device

I would like to create a form for a survey. but what I want to do to send out surveys when I upload a file from excel. How will the survey send out a survey from my phone. so the clients can do a survey.

First off, the forms you create at JotForm are mobile responsive so they should be easy to fill on mobile. If you want to allow file uploads in your form, you can simply add the File Upload field to your form. Guide: https://www.jotform.com/help/332-How-to-Create-File-Upload-Forms

Once your respondents submit the form, you should receive email notifications and as well manage the submissions data on your form submissions page.
