Already registered but still cant get the form.

I signed up two times and the last time I was able to print the form. I registered a week ago for today's testing, but still cannot get the form. Can someone assist me with this issue?

I am not sure I follow, were you trying to fillout a form?

If so, can you share with us the Direct Form Link of the form you filled out?

Upon checking the account there seem to be no forms in it.
