Can you please confirm if you've made any changes to the spreadsheet file? Please note that renaming a header in google sheets or renaming a field label on your form will result in a broken integration connection.
You can try to reintegrate the form to google sheets again and see if that will fix the issue: How-to-integrate-forms-with-google-sheets

I did rename it. I wasn't aware that this would cause a break. How can it be fixed?
