Hello. We have set conditions on a covid consent that if they choose "person with chronic condition" that it shows a "please specify chronic condition" and that this field is required. This field is coming over blank to us. And it's not even showing up in the list of columns either selected or unselected in the view submissions. I tried to add the column to the display but it's not even an option. Can you please see what's going on? Did we do something incorrectly or is this a glitch? Thank you.

Hi there M8240,
Thank you so much for that information. I have created a clone of that form, and I believe I have found your issue. You are using conditions to hide things at the very start of the form. We can change lots of these fields to be hidden from the beginning and that will simplify things quite a bit!
To change a field to hidden, click on the Properties button of the element and then select the Advanced tab. From there, scroll down until you see Hide Field. Toggle this ON to make the fields hidden from the start.

Please hide the fields with the following labels:
- Medicare Part B Number:
- Please specify chronic condition
- When did you receive your first dose?
- In order to have your vaccine administration fee paid for ...
- Social Security Number
- State identification number and state of issuance
- Driver's license number and state of issuance
Then, set the following field to required:
- Please specify chronic condition
Next, please delete ALL of your conditions. Please replace them with the following conditions:
In the condition above I have included all ages from 65 up to 125, as the oldest living human is 122. You may copy the text from here: 65,66,67,68,69,70,71,72,73,74,75,76,77,78,79,80,81,82,83,84,85,86,87,88,89,90,91,92,93,94,95,96,97,98,99,100,101,102,103,104,105,106,107,108,109,110,111,112,113,114,115,116,117,118,119,120,121,122,123,124,125
The above condition is similar to your old one, however we will select all fields from 19 to the end.
This last condition you may not want to recreate, but I have included it because I am not sure if you had a reason to not collect the insurance card photo.
Once you have made these changes, the form should behave as expected.
