To my understanding, you would like to combine two fields that will populate from 2 different parameters on your spreadsheet? If it is, I think this is not possible for the widget to combine 2 data from different columns on your spreadsheet and populate a single field.

What you can do is to add a Form Calculation widget to concatenate the data from two fields:
