How can I add a PDF file with terms and conditions to my form

I have created my form and I need to add terms and conditions for guests to sign. Please help

If you wish to add terms and conditions to your form, you can use Short Scrollable Terms or Terms & Conditions widgets. You can also add them to your PDF Document.

Thank you . I have my own terms and conditions written , in word. I need to upload them into my form. This is the support I need. To convert my work terms and condition written in word and include them in my form as a PDF file. Please look at my form and advise
