Regarding your pricing, is there a cost per email to send a survey via a blast? So would sending to 4,500 users to get survey responses cost per send, or would your plans cover this cost?

Jotform doesn't have any marketing built in platform so the limit would be based on what your email blasting has. You may integrate some of the email marketing apps to Jotform for free like MailChimp, GetResponse etc..
