Creating a form like I've used on someone else's site

    Hello- I have used a form on someone else's site and would like to make one like it for myself. I have included a screenshot. The way this is setup is that when you click a number, a description comes up specific to the number you clicked. At the end of the survey, a total is added up and shown, and a review of your choices are also shown. Could you point me to the widgets I would need to do this with?     Please try using our Card Forms. Here's a guide: https://www.jotform.com/help/491-how-to-create-a-jotform-card. Would that work for you?

    Sorry but this answer did not help me. I'm looking for one specifically like I sent a screenshot of. Is the one in question specially coded, or is a template that you have already established?

    Hi,

    if you have the link to the form, and the form owner hasn't disabled this option, I think it's easiest if you clone the form and modify it to your liking: https://www.jotform.com/help/42-how-to-clone-an-existing-form-from-a-url/

    The form most likely uses conditions, and a specific template, but we would need to look at the form to give you more specific instructions.
