How can an email be attached to the forms being sent back, so I know who it is from?

I am using this link, but when receiving the forms back, it does not indicate the email address who the reply is coming from.

Are you referring to the reply-to email address?

If so, you can change it in your recipients tab:

You can also change it to your SMTP in the advanced tab:
