How Can I Create an Online Vendor Application for My Nonprofit Client

    I have attached a jpg of my nonprofit's current hardcopy form. I want to create an online version of this form.

    One particularly important feature is the ability of someone completing the form to be able to select those dates they will be able to vend. The current form asks them to circle those dates. I am hoping your forum builder is able to accommodate this feature.

    You can use our Smart PDF Forms to convert your PDF into an online form.
    With Smart PDF Forms, you can also save the collected answers on a same format as your original PDF.

    When you import your PDF to the Smart PDF Form, sometimes the Form Builder does not recognize all of the fields correctly. In this case, you can manually add needed fields on the form. To do this, please use this guide: How-to-link-your-online-form-fields-with-pdf-through-smart-pdf-forms
