Reviewing the form on your account. I can see that you have not yet completed the setup of your form's Email Alert. Please access it and fill out the Sender Name ( A field on your form that defines the user's name) and the Sender Email Address (email@example.com is the preferred option). Without these fields set up the Email Alert will not be sent out.
