Submit button not working

    Here is the form embedded on my website. A client told me she couldn't get the Submit button to fire, and I'm having the same issue. This is a form that's been working like a charm. What do I need to fix?

    I have tested your form on IE 11, Firefox 29 and Google 35. So far I have been able to submit on all three without any problems, please provide the nme of the browser you are having trouble with. So that we may look into this further.

    In any case I would suggest to re-embed the form using the iFrame code provided:
