Thanks for reaching out to Jotform Support. To answer the question in a number or a rating format we have Star Rating and Scale Rating elements in our form.

As per your screenshot, I am unable to understand your requirement correctly. Could you please provide a little bit more detail about it also please share the form link and let us know on which field you want to add the rating option. So we can look into the form and try to understand your requirements accurately.

Hi, thanks for the quick response.  I need to enter a ratings scale as per above examples into my form.  I have attached the forms with the questions I need to have rateable. How can I attach a form now?

Unfortunately, the forms you have attached are not visible in this thread. You may just post the URLs of the forms that you need the Scale Rating function applied.

To create a Scale Rating field with a table format similar to what you have posted previously, you may instead use the Input Table field. You may refer to the screencast I made below.

Thanks for getting back to us. I can see that you have created a Smart PDF Form for this. Based on the information I can see on your Original PDF, you would like to calculate a score depending on the user's answer. What I would suggest is to add all questions to the Input Table, assign a Calculation Values for each Column and use a Form Calculation widget to calculate the value for the total score.
