Forms Printed for fill out with all options shown as online form

How do I print the forms to fill out for persons that do not have online access and does it show all my options please see my form to understand. I need everything to show on the print out form as it does online. is there a pdf version i can print out because the one i tried showed the print out looking weird. is there a live online chat support?
