Notification and Autoresponder content not correct

    Hello,

    I recently created a form and encountered some issues when testing it. While the form is correct when accessed through the link and the submission worked well, the subsequent emails are incorrect.

    The notificaiton email that I get when the form is completed duplicates a previous (multiple choice) section of the form and does not show any of the responses of a second (multiple choice) section.

    The auto responder email has the same problem but it duplicates the section that was missing from the notification email.

    Any help would be much appreciated.

    Hello 5414serviceunitmanagers,

    It looks like your Multiple Choice fields have the same Unique Name - please change the Unique Name of one of them to some other name:

    After doing so, you can remove the email templates and add them in again to refresh them:

    Alternatively, you can manually edit the templates to include the new Unique Name:

    Thank you so much! That fixed one problem but seems to have created
    another. Now I am not getting any of the emails. Either as a form user
    (when I fill it out and sent to my personal email) or as the form owner (to
    the email used in our jotform account). Any help would be much appreciated.

    Hello there,

    I cloned and tested your form on my end and I was able to receive both notification and autoresponder email. I assume that this form is affected by the known issue on our server a while ago.  We received feedback from our developers that the issue is fixed now.

    I didn't realize that there was a server issue. I apologize but I still
    have an issue. As a form user the first email I receive indicating that my
    responses were submitted shows the correct items for the multiple choice
    checkboxes. The SECOND email I receive as a form user saying my form has
    been approved displays the duplicate on one question and none of the other.
    Similarly, as the jotform owner, I receive one email telling me someone
    completed the form. That correctly displays the information. The second
    email to the jotform owner saying your approval is required shows
    duplicated information.

    Hello there,

    It appears that you're referring to your approval flow emails. Can you please delete and then recreate your approval flow?
