Locate the account that has the subscription to cancel

    I cancelled these services with Jotform, but you sent a debit to my bank account. I cancelled this service when I closed my business in 2021. Please do not debit account, thank you.

    Thanks for reaching Jotform. We’re sorry if you’re having inconvenience with this. I checked the email account that you have used to reach us but it seems that this is a Guest account only. Can you share with us the email address of the Jotform account that you used before? If you still access the account, please open another ticket to request the cancelation of the subscription because only the Jotform account itself can request the cancelation. If there are difficulties in accessing the account, you can follow the steps on resetting the password on this link.

    In case you can no longer access the account and you cannot remember the account, please provide the following information and we will raise this to the Billing Department to locate the account and cancel the subscription then process a full refund if you were already charged:

    The last four digits of the card:

    The expiration date of the card:

    The full name on the card:

    The type of card (Visa/Mastercard/Amex):
