Form field widen when mobile responsive widget is added to the form

Hi I have a form located at

http://www.unitedmarkets.com/2014/survey.htm

and trying to add Mobile responsive widget but having trouble. Somehow it is changing a size of my form fields.

But I created another form located at

http://www.unitedmarkets.com/2014/job.htm

and installed mobile widget and had no problem. But can not seems to be working with http://www.unitedmarkets.com/2014/survey.htm

I like your customer support so I did sign up for 2 years. Hope to give you lot more business .
