Sending notification to a specific email

    I have my form set.  It has an initial page and for subsequent pages, depending on which option is chose on the initial page.

    I am trying to set it up so that when I hit the Submit Button, email will be sent to a specific address.  I can get that to work, but the challenge is that it is not consistent.  I do a test (by filling out the form), hit Submit-- works great.  I  create a new Safari page, open the Form again, do the same thing and then it sends the form correctly to the appropriate address, but then also sends me an email to my direct address (which IS one of the options, but not for the current page).

    I did try to create a Radial Button with Conditions.  The hope was that a person would click on the "I Agree" radial button, and I thought it would then send the email.  I hit the button, but nothing happens.

     

    I am close and this is working great all except for this.  I am sure is operator error, but I can't figure it out and I did try looking through the online help first.

    I checked your form and you have three different notifications:

    As there are no conditions associated with the email notifications, it means that every time that the form is submitted, three email notifications should be sent.

    I made a test submission. Can you please confirm me if you received?

    If not I kindly ask you to tell us which email is causing troubles.

    The emails came, but went to the wrong email addresses (or multiple email addresses).
    The section for On-Site Live Recording is supposed to go only to:
    gary.erickson@greenville.edu
    The sections for Live system and Blackroom Reservation are to go to:
    gcaudioservices@greenville.edu
    The InStudio Recording Sessions is to go to:
    gcstudiomanager@gmail.com
    I was a 1,000% sure that these had conditions, but maybe what I remember was that I was trying to see if I could add a condition to the "I Agree" button and was planning to change that to a "I Agree and Submit" button. When I did that, I would fill out everything and then clock on the I Agree Button (a Radial button with only one option) and nothing happened.
    I did try to put a condition on a Submit button, but did not see how I could do that (it would not show up in the list of available options in the "If (please select) Field". If we can get the "I Agree and Submit" option to work, that would be fine (as long as it can go to a "Thank You" page as well.
    Does this info help, or do you need additional information or clarification?

    I created a clone and added the requested conditions. Right now with each submission it sends only one email notification according with the conditions. The emails were sent, so I believe the situation is solved:

    You can start using the clone form (that is already in your account) instead.

    But as you have 4 options in the what service field, it is necessary to create an extra email notification, else when the 4th option is chosen you won't be notified.

    Thank you! I see where you put the Conditions now, I was trying to put them on the Submit button or after the form was filled out- you set it per section.

    one last question-- is it possible within the JotForm to change the name of it? For example, instead of:
    http://form.jotform.us/form/42874753851162
    can I name it:
    gcaudioservices.com
    to be more fitting to what the title of the service is?
