Form ID did not match error message

    I'm getting this error message: Form ID did not match. Form ID: Form Name: 4273555435515

    I could replicate the same situation:

    Also it seems that the styles are not working, so I believe there is a conflict of the form with the site:

    Please try embedding the form again, preferably using the iFrame method.
