I would like to know if it is possible to change/adjust the email structure that is sent to me via JotForm. I have attached the - Where the form is - AND - What I want it to look like.
If it is possible please let me know, and where I may go to make the changes.

It is possible to edit the email notifications:
But if you edit the email notification they will stop update automatically if you edit (remove/add) fields to your form. So if you edit the email notification it is necessary to add/remove manually the fields added to the form.
I would suggest to create this structure on a word doc and then copy paste into the email notification editor:

After pasting the structure it is necessary to manually add the fields by clicking on the right menu:
