Thanks for your suggestion. However, when I moved the fields over, I lost the whole conditional functionality that I needed.

Unfortunately that is how the Field Manager works. It is not capable of moving the existing conditional statements of the fields between the forms.
A workaround you can use is clone the form itself instead.
User guide: -How-to-clone-an-existing-form-from-your-account
When you clone the form to your account, it will carry also the conditional statements.
You can manage the cloned version of the form using the Field Manager Wizard.

It will be easier to manage the fields and conditional statements with the cloned version of the form also.
