If I did not misunderstood your request. Regarding on the map, you can set that on your form submissions settings by clicking on the small gear icon found on the upper right side of the submissions page. Check this screen.
Do you mean, you are not getting users answer on the notification message that was sent to your email after form submissions? If so, recreate your form notification, justdelete your existing notification and create a new one. That should fixed it.
