How to put in part in form for items and quantities

I have 3 items on my t-shirt order form and  just need to put the item name and quantity drop down box with some text underneath with item description or notes.

I can't figure out how to edit the one on your template with t-shirts, shoes, etc. It only lets me change the text box on the left.

Then how does it connect to pay pal?

Since you have entered your PayPal email address, the form is already integrated with the PayPal service. The form users will be redirected to the PayPal payment page upon subissions.
