I would really like to speak with someone over the phone. I have invested time and money into switching over from form central to jot form and I would really like to ask someone the
questions I have regarding e-mailing my survey through constant contact and other questions I have regarding jot form. -

Unfortunately, we do not have over-the-phone support at this time. You may contact us through this support forum and on our support email at support@jotform.com . And we will attend to your concern as soon as we can.
As to your current concern, I checked your jotform account/username deltaartsalliance but I did not find any issue with the account. It is Active and in good standings.
When I checked your My Forms page, I was able to see your forms available.

Can you tell us the actual name of the missing form? We will search for it if it is missing on your My Forms list.
