How can I have a notification email only include fields that were entered

    We have a form that has fields for three departments. We would like to program the notification emails to ONLY shot the fields that have been entered when someone fills out the form. See below for example the fields marked out in red were not entered because they are hidden fields. How can we get them not to display in the submission? thanks!
