Thanks for your response. To clarify, We're actually using JotForm to create a New Customer Application. I have the form completed, I just need to figure out how to link it on our opencart website. The website is at:
At the top right of the page there is a "Wholesale Account" link. we'd like that link to go to the new Jotform application page. Presently, it links to the Contact Us page. We'd like to keep the Contact Us page intact.

Can you clarify that you still wanted your current Contact Page to remain? Or you meant you want to replace it with your JotForm Contact Us form.
As I mentioned also previously, you can embed the JotForm into your Contact Us page on your website.

Yes. We want to keep the Contact Us Form, as is.
We just need to figure out how to change the "Wholesale Account" link at the top right of the page to go to the new Jotform page instead of the Contact us page. probably somewhere in the Opencart back office.

One other question. How do we change the email address that the form submissions are directed to? I used a test email address when creating the form, and now I want to change it to the live email address.

Glad to hear it! Normally I would split your new questions off to a thread of its own but since it is quite easy to change I'll go ahead and include how. Please refer to our user guide on How to send notifications to multiple email addresses
The process of changing a single recipient email address on your form notifications is just the same on that user guide.
