People are complaining that they have submitted our Pet Adoption Application Form several times, however, we are not receiving them and they are not coming into our Submission box. I checked the "Check Bounce List" and we are not on it.
Also, please remove the Pet Adoption Application from the Templates, we are still getting wrong submissions. Pet Adoption Application This template should have been removed almost a year ago, as requested. It still has a WHDR code in it.Please address these concerns immediately, the rescue depends on people for fostering and forever homes for our loving pets.

Thank you for contacting us.
The form will not trigger any emails if the entry is not added to submissions (it means that we have not received it).
Do you have that form embedded on some web page? If yes, please provide us with the URL.
The second concern has been moved into a separate thread, so that we will be able to address it properly.

I checked your account and there are several submissions in the submissions view for your form:

Your notification is set to be sent to "RKLARK@VERIZON.NET". Make sure this is the correct address and that it is entered properly. Try the address without all caps.
Also, I noticed the page your form is embedded to is not properly formatted:
You may want to add a doctype declaration, as well as head and body tags:

I have read your response. The is email address is correct. When you say the "page your form is embedded to is not properly formatted", can you explain exactly what you mean? They were coming through before, so did something change on your front or ours? We have done anything different, so I'm at a loss for what has happened. Would it work better if I didn't embed the form? Is there anyway you can give specifics on the right way to add a page and this form so the email/submissions come through?

I did check your webpage where you have embedded your form and it seems you have added your form's script embed code within your webpage's head tag. Please check the screenshot below:
This is correct that your embedded form is not working correctly. The validation of your form is not working as well. I was able to submit your form without any input data. Please check the screenshot below:
I would suggest you to remove the script embed code form the head tag of your webpage and add embed your form's iFrame embed code within the body tag of your form. one of the applicants reported that their application indicated that it was cc’d to “adoptadal” email address. Any ideas on this?

No, we only have one recipient on our form.

As far as I can tell, the only address your notification will be sent to is "dalmatianrescue@comcast.net". If your notification was set to be sent to additional addresses, all of your notifications would show the same information. The notification may have been CC'd by the original recipient.
