Website Not Loading after Embedding Form

When I checked the page I can confirm it indeed isn't loading.

I would suggest trying to re-embed it within an iframe code to see if that will help resolve it.

More than likely there's a script running on your site that is interfering with the form and vice versa so an iframe would prevent that problem from occurring.
