When this form is submitted, the pdf file that drops into Google Drive is only the 1st page, so it doesn't show all the data submitted. Are we overlooking something or is there a way to correct this? If it's a glitch, it's obviously significant! Really need a reply to this soon! Thanks!

I've cloned your form and integrated to my Google drive but I couldn't replicate the issue. See screenshot below.
Can you try re-integrating Google drive to your form or clear your form cache and see if it solved the issue?
