Change uploaded file name to match the fields from the form

Is there any way that uploaded files can be renamed according information input into the fields?

Unfortunately this is currently not possible. I recommend you integrate your form with DROPBOX, configuring different folders per submission. http://www.jotform.com/help/77-Upload-to-Dropbox-with-JotForm?
