How to create a recurring and one-time payment form ?

I have attached a payment form to show you what I am trying to do.  I am trying to incorporate a one-time donation and a recurring donation into one form.  I have read on your forum that it is possible to do this.  However, I am not sure if I am going about it the correct way.  As you can tell from the form I uploaded I want to give people the option of choosing an amount and then choosing if they want that amount to be recurring or one-time.

I may be going about this wrong but it seems as though I will have to list the amount for each payment frequency (monthly, quarterly and annually).  See my sample that is attached?  Is that the right way to do it or do I need to set it up differently?

Also, can you tell me how the variable price field works in the subscription module?  I am trying to see if I should set up a field like this or some how use an OTHER field so people can write in their pledge amount for a recurring payment.

Also, will we be able to set when the first payment should begin?
