How to ensure my text and header fields print or show when someone prints my form or sends it to me?

I have looked into this repeatedly and I haven't been able to figure out how to ensure that people can fill out my form and when they are ready to print it doesn't just print the questions on my form. Also, when I receive a submission, I am only able to see the questions from the form - not the entire form. I need to be able to see the headers, form, questions, images, etc - so does the individual when they print.

To ensure that your Headers and Text show up in your printed submissions you would need to make sure you have enabled the headers and text to fix that problem. This part then has to be printed out via your submission viewer to print it there or using the pdf option to view and print it that way.

As for the formatting of when they are sending it to you you would need to either enable the attached pdf option or add it within the email template since the email notification only shows you what you have set up in there. You could even go far as to reverse that by sending them a pdf copy via an autoresponder containing the submitted information and headers and text shown there too.

Alternatively, you could try this - https://www.jotform.com/help/335-How-to-print-a-form-exactly-as-it-is-when-viewed-but-with-the-user-entered-data which should help.
