I checked your jotform http://www.jotform.us/form/60885330766160 and I think you have fixed the issue already.
The previous Notifier must have been corrupted and was not updated with the current fields on the form. Since you created a new Notifier on the form, everything is now updated.
I see your test submission resulting Notification Email to have complete data.
Let us know if issue still remains.
