    Can you please elaborate a bit more on what your issue is. When reviewing your site, I noticed that you had used a JavaScript to embed your form.

    Are you trying to embed your form or trying to create a feedback button, to show your form

    If you are trying to embed your form, we recommend using iframe instead of Javascript. Using iframe will eliminate many conflicts that you could have between your form and site.
