How can I download a blank form? I need to use it for older people who dont have internet.

I have several elderly patients who don't have a computer.  I need to be able to download my blank form so they can complete it on paper.  Suggestions?

I am not sure whether you are asking about downloading the form (having a digital copy of the form), or printing it out on paper.

If you are looking to download a digital copy, you can simply create a PDF form out of your JotForm form:

If you are looking to print the form on paper, there are multiple ways to print your form.

1. Probably the easiest will be to add a Print button to your form, and then use it to Print out a copy:

2. Alternatively to adding a Print button to your form, you can simply open your form through its direct link, and then use your browser's Print button in order to print the form to paper.

To get your form's direct link, if you are on your My Forms page, simply click the form, then More > View Form:

When the form opens in a new browser tab, simply print it out like any other webpage - by pressing Ctrl+P, or by using your browser's Print button:
