The current issue is how to configure the data collection table.

    See https://www.jotform.com/?formID=61622529852863

Column 1 is text that cannot be overwritten.

Column 2 is Numeric data entry

Column 3 is Numeric data entry.

This is an element of a 28 item questionnaire to be sent by email with the data collect via your system for each individual farmer Subscribing.

I plan to email this data form to our clients subsequent to a request for the service. Early client base perhaps 100 per annum expanding to 1000 in time

Thank you if you can direct me on constructing your sheet

    Regards

Please take a look at the following guide on how to set up the Configurable List widget:

    https://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget

As described in that guide, if you are looking to have static text and two number fields per row, then you will have to set up your configurable list widget with Static and Number fields.

However, based on what you are describing, it seems like you will always have a fixed amount of questions per form - 28. Configurable List widget is meant for instances when you do not know how many entries you will be receiving, so that your customers can create as many rows as they need (and you allow them to).

Since you are using a questionnaire with the same 28 questions for every customer, you may want  to use a regular Matrix field instead, which is found under Survey Tools:

Simply use the Rows, Columns, and Input Type buttons to set its "Periods", questions at the top of 2nd and 3rd column, and Numeric field types respectively.
