Form is stuck on "Please wait"

I've added my form to our website and it won't leave the please wait.  It stays until I leave the website.  What am I doing wrong.  :)  Help please.

I’m so sorry, I didn’t investigate enough before I sent the email. I found an answer on your support page. I needed to use the Iframe and it worked.
