    I have checked your form, but could not find an integration with Facebook.

    I think it was due to script conflict that is happening between those on your website and on the form.

    I suggest you do a re-embed process. That is, remove the currently embedded form on your website, save your page, then embed again the form but this time use its iframe embed code.
