How to provide time slots on a form for making appointments

    We are a non profit agency supporting victims of domestic violence. Every year a donor puts together Thanksgiving boxes (one turkey, stuffing, cranberry, pie etc) for the clients. 

    I need to create a form that allows our Client Services Advocates (case manager) to sign up for specific time slots over two days.

    I am envisioning a calendar of some sort where they can pick their time (obviously with all their other information which I know how to do) but when the time slot is filled (three CSA's per hour time slot) it disappears from the selection area, or greys out, or is otherwise noted as unavailable. 

    Help please! I can't figure this out...

    We have other similar widgets where you can limit the number of times your users can select an item, such as our Inventory, Gift Registry, or Quantity Gift Registry widgets. However, I would first recommend trying the Appointment Slots widget, and seeing if that one will work to your satisfaction.
