Calculation Fields Are Not Showing Up On Form

Hello, the 1 week calculation and the 3 day rush calculation are not showing up when we publish this form.  Can you please assist on why it is not displaying correctly.

I checked your form as I see name of the Calculation fields not {calculation1} and {calculation3} you can check names of the fields by clicking properties. Also here is the guide that might help you to find field names.

Please note that you hided most of your fields and you can not see those fields on your form because of this reason. 

If you want them to be shown on your form, you need to click "Show Field" option in properties menu of related field as just you can see here.
