Need help in creating a questionnaire with few calculations based on the answers

I have this following need. Can you tell me if this can be done with Jotform:

A want to have the client fill a questionnaire. The questionnaire template will have questions such as this one:

On a scale of 0 thru 100, what is the level of stress due you experience: [client types in a numeric value from 0 thru 100]

There will be about 15 such questions.

Once the client clicks submit, I want to do the following:

Do a few math calculations based on the answers. A weighted averaging of the answers.

Send the result to the client as well as to myself. 

Can this be done.

It looks like your requirement is achievable by using the calculation widget. You may add the number fields to the form as your question fields.

The number fields can be restricted with minimum and maximum value from the field properties.

Now, add a calculation widget to the form to calculate the result as per your requirement.

You can add the question fields to the calculation wizard and build the formula as needed.

Since you want to receive an email once the form is submitted, you may add an email notification to the form by following the guide Setting-Up-Email-Notifications. If you want the form information to be sent to the form filling person, you may add an email address field to the field and then create an auto-responder.
