My form is not presenting all sections for completion?

My form was working fine until I made some changes. Now it does not present all the pages for completion. It acts as if logic is applied and it skips all the way down to the conflict interest section.

Can someone take a look at this for me and tell me what occurred and how I can fix it?

Your form has 5 pages. Conflict interest section is on 4th page of your form. If I understood you correctly page 2 and 3 are skipped when you test your form. I have tested your form, but I was not able to replicate mentioned issue. I was able to fill fields on all five pages of your form. Pages 2 and 3 were not skipped.

I have also inspected your form and you do not have skip to page conditions set inside of the form. You have only three conditions for fields on 5th page of your form. 

You also have condition for fields that are deleted from the form. I would suggest you to delete that condition.
