Two Part Question Field

Is it possible to create a two part question within a form? Or what would be the easiest way to set up the below...?

How much did you spend on your last visit?

$____ total for ____ people

(check box) I don’t remember 

That is possible to achieve with Narrative Fields widget.

However check box option can not be added inside of this widget. You would have to use separate checkbox field.
