    Can you help with this?  I'm losing business. I didn't set up the form and don't know much about how it works.

    I have tested your form and I was able to replicate mentioned issue. I have filled all fields on your form, but when I submitted the form Incomplete Values error message was shown.

    After that I have tested your standalone form https://www.jotform.com/form/72170119334 and i was able to submit it without any issues. Once I submitted your standalone form I got redirected to Thank You page.

    Form on your website is embedded with full source code. I have also checked and you form was last time modified more than two years ago. It is possible that full source code is outdated. I would suggest you to get new full source code of your form and re-embed it on your website. Or you can re-embed your form with iFrame code. inside of this guide you can find how to get iFrame code of your form:

    Form that you provided is created on account with username lisamc. If you do not have access to that account I would suggest you to contact the person who created and embedded your form and ask him/her to re-embed your form.
